How to register

To register with the practice a registration document can be downloaded via the link below, alternatively you can register online.

Please attend the practice from Tuesday onwards between the hours of 10:00am and 6:00pm in order to register at the practice

PLEASE COMPLETE THE FORM BELOW AND BRING IT WITH YOU WHEN YOU REGISTER

You will also be asked to provide as per practice policy;

  • Your medical card (from PRF1 to be completed if this is not available)
  • Photographic proof of identification – driving license, passport
  • Proof of address (dated within 3 months)
  • If registering children under 10 years of age, please provide copies of all immunisations

Once all registration forms have been completed you will be given a health check appointments with our health care assistant. This is the opportunity to obtain important medical history until your notes arrive from your previous GP.

Please note

New patients who fulfil government criteria of eligibility to NHS care are able to register with this practice, after an application process, with this practice.

  • All new patients over the age of 18 years will be offered a health check appointment with the practice nurse
  • Patients over the age of 75 are seen annually in the surgery or visited at home
  • All patients of all ages are to have a Named GP

If you have had any previous medication, or have had a change of medication, remember to bring copies of your green form, which is attached to every prescription, on request.

Please bring a friend or relative who can help you to complete our registration forms, if you are unable to speak the language or have a disability.

Find Your NHS Number

Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.

This online tool will help you find your number if you do not know it.

Please Note

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Other ways to get your NHS number

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter
  • call your GP surgery and ask them for your number

Temporary Patient Registrations

If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.

To register as a temporary resident you will need to contact the surgery and complete and return this form

You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours, but less than three months.
